Home-from-hospital and Rapid Response
Your Care Support Worker can help and assist you when you come out of hospital or if you are recovering from an illness, even overnight. We will work with you to regain your strength and independence by providing advice, guidance, information, and hands-off support rather than doing things for you. This process is sometimes known as re-enablement.
We have developed a rapid but thorough assessment process to aid your return from hospital back home, ensuring that you are safe and that your wellbeing is our priority. You will be given one of our 'Welcome Home' packs which include essential and emergency provisions.
